The Centers for Disease Control and Prevention (CDC) have reported over 2,318 measles cases in the U.S. this year, overtaking the record number for all of 2025, which stood at 2,289. With five months remaining in 2026, this year has already become the worst for measles in the U.S. since 1991, marked by significant outbreaks in states like South Carolina, Utah, and Arizona.

Since the U.S. eliminated local measles transmission in 2000 through high vaccination coverage, there's a growing concern that the nation might lose its measles-free status. An international meeting in November will seek to confirm if the U.S. and Mexico have failed to maintain this status, as vaccination rates continue to decline.

As seen in Texas, where the 2025 measles outbreak is still being analyzed, early estimates suggest the numbers could be nearly double the reported cases of 762. Mathematical epidemiologist Rémy Pasco outlined that inadequate vaccination levels result in inevitable miscounts during outbreaks. The CDC defines an outbreak as three or more linked cases, but in low vaccination areas, identifying these linked cases significantly falls short of reality.

Texas' chief state epidemiologist, Dr. Varun Shetty, expressed concerns about untracked cases within households, illustrating the disconnect between reported and actual cases. The state health department has previously reported instances where approximately 182 probable cases in a key Texas county went unconfirmed. Dr. Shetty predicts that without increased vaccination efforts, future cases and outbreaks are likely.
