Michigan is currently facing its most extensive outbreak of cyclospora, a parasite known for causing prolonged watery diarrhea, with cases surpassing 1,000. This marks both a significant public health event for the state and one of the largest outbreaks seen in the U.S. in years.
Health officials reported that as of now, no fatalities have been linked to the outbreak, which initially began gaining attention last week when the number of confirmed cases reached 170, all concentrated in the southeastern region of the state. Historically, Michigan averages around 50 cases annually.
By Wednesday, those figures climbed to 992 confirmed infections in Michigan, with around 40 individuals hospitalized due to severe symptoms. Across the border in Lucas County, Ohio, an additional 306 cases have emerged, contributing to a regional health crisis, with northwest Ohio also reporting over 500 cases.
Dr. Natasha Bagdasarian, Michigan's chief medical executive, emphasized the interconnectedness of these cases, stating, "There is clearly a linked outbreak happening right now." Investigators are looking into food sources, though identifying the origin of cyclospora outbreaks can often be complex.
Understanding Cyclospora
Cyclospora is a tiny, round parasite that frequently leads to gastrointestinal distress, resulting in symptoms including “frequent and sometimes explosive bowel movements,” as noted by the U.S. Centers for Disease Control and Prevention (CDC). Typically, the illness, known as cyclosporiasis, is not life-threatening and can be treated effectively with antibiotics. Outbreaks of this parasite are observed to peak in late spring and summer months.
This heat-preferent parasite spreads via fecal matter and has been traced back to contaminated food, particularly fruits and vegetables irrigated with tainted water. Compared to other foodborne illnesses like salmonella and E. coli, cyclospora is less commonly reported, but its incidence has risen in recent years, partly due to climate variability and improved detection capabilities.
Comparing Past Outbreaks
While comprehensive data on cyclospora incidents is sparse, it's evident that only a few outbreaks in the last two decades have exceeded 1,000 cases. Some notable instances include a 1997 outbreak linked to Guatemalan raspberries, which affected over 1,000 people in the U.S. and Canada, and a 2019 outbreak connected to Mexican basil that made more than 2,400 people sick.
Melanie Firestone, a foodborne illness researcher from the University of Minnesota, cautions that many of these cases go underreported due to limitations in testing methods that do not specifically target cyclospora. Compounding this issue, the parasite cannot be cultured in laboratory environments, thus making it harder to trace cultivated foods back to the source. Notably, the complexity of food distribution further complicates these investigations.
Current Health Status
The outbreak is primarily centered in southeastern Michigan, although it's not classified as a national health emergency. The CDC indicated that there's no current evidence suggesting that cyclospora has mutated to become more infectious. Data shows that thousands of cyclospora cases are reported across the U.S. annually; however, the incidence this year is already four times higher than similar data from the previous year.
While Michigan is currently bearing the brunt of the outbreak, Dr. Bagdasarian suggested that state officials' proactive approach to investigating and reporting may contribute to the perception of a localized problem.
Preventive Measures
To mitigate the risk of cyclospora infection, individuals experiencing prolonged diarrhea should seek medical advice, particularly if symptoms persist beyond a few days. Preventive measures highlight the importance of avoiding potentially contaminated food or water sources.
New recommendations from Michigan health officials include washing fresh produce thoroughly before consumption and opting for whole heads of lettuce over prewashed varieties. If using leafy greens, they advise removing the outer leaves and rinsing the inner leaves under running water. Cooking vegetables may also provide an extra layer of safety.
